Take the next step and meet me for a Telehealth Visit!

You have read my book, you tried my homework and you have a question that they don't answer. Or perhaps you have a hot new injury and you want to do it perfectly, right from the start. I have your back my friend! I am more than happy to fill in the blanks to ensure you heal properly and am so excited that you are interested in taking the next step in working with me. What an honor!

Together we will delve through your injury, like organizing a closet. You just need to show up to work, and put in the effort and you will heal from your injury faster and better with my process.

We start with a face to face and you can tell me what you need and your injury story. My game-plan will be clear, concise and easy to follow. I'll also give you a downloadable training plan to fill in so we can evaluate your progress. As for how many visits you will need, sometimes we need 3-4 visits, but many of you get what you need out of just one!

Lets begin with these easy 3 steps to get you in the door:

1. Book a Visit.

As my office is simple and streamlined, all booking are done through my online 24-Hour booking portal. Take the stress away with knowing that you can book at anytime, from anywhere. As a new client, book a virtual visit and if you have advanced imaging, please choose the corresponding appointment.

If you need advanced imaging such as a Diagnostic Ultrasound or X-ray, we will discuss this at the time of your visit and if appropriate, order one with your local clinic. After imaging, a follow up Telehealth visit will be booked so we can read these images together if needed.

NOTE: I also see in-person clients at my clinic so please make sure you are booking the correct visit ;-)

2. E-Mail the following forms:

Please email them to drlisabrindc@gmail.com

3. Pay online. (Email Insurance Info if desired)

Payment is made automatically via my secure online system at the time of booking.  I am happy to submit to your insurance however I usually submits how much you paid us (and we see if they reimburse you!). As an 'Out of Network' Practitioner, be informed that the deductible is usually different than in-network.

Insurance Note: Almost ALL insurance companies will not reimburse care without your deductible (In Network or Out of Network) being met first. For those of you with backup insurance, please also email a copy of it to drlisabrindc@gmail.com. We can bill up to 30 days after your visit.

The Specialist Copay on the front of your card is only taken at my office when you have met your deductible. All others will receive a superbill upon request for submittal to insurance.

Work Accidents (Workman's Compensation)

If you have been in a work related accident and wish to bill your insurance, please ensure it indeed covers our care. Your care is automatically covered in most cases without question as long as it pertains to injuries sustained in your accident. I cannot bill your health insurance if your injuries are sustained from a climbing accident.   If this is you, please show up to your visit with the following:

    - Claim Number

    - Adjustors Name and Email

    - Insurance Phone/Fax

    - Date of Accident

My system is designed to receive payment during booking. Please notify me at your visit if you'd like your auto insurance billed instead and we can modify payment at the time of service.

I am happy to work off of a Doctor's Lein and to wait for payment until your case closes. Upon closure, payment is due within 30 days whether your insurance decides to cover it or not. Please fill out this paperwork and bring it with you.

FlexSpend and HSA Cards

All credit and debit cards as well as HSA and Flex-Spend cards are welcome in our office. If you are having trouble booking (with or without pre-paying online) with your FlexSpend or HSA card, please call and we will book for you. Alternately, you can book with whichever payment card you like and we can modify this for those whose HSA or FlexSpend cards don't allow online payments. Your company sets this up for your safety and we can work with them easily in most cases. Care is covered under all FlexSpend and HSA plans.
